| All services provided are non-sexual! Reservations My calendar fills quickly as I am a sole practitioner in a studio, not a day spa. Since time slots are limited (12 to 20 per week), I suggest booking your appointment as far in advance as possible. Please call to schedule a minimum of one month prior to your desired appointment, as I am currently booking up 2 to 3 weeks in advance for daytime appointments and 4 to 6 weeks for evening appointments. I strongly urge you to schedule your next session at the time of your appointment. (Occasionally, I can accept same-day appointments, but I typically require a minimum of 2 weeks notice). I will be happy to place you on my waiting list and if I have any cancellations I will give you a call as soon as possible. Appointments may be confirmed by phone call the morning of, or the day before, your session. Arrival I schedule 30 minutes between the end time of one session and the start time of the next, which allows me to prepare the room for the next client and provides a peaceful and unhurried environment. Please arrive 5 to 10 minutes prior to your scheduled appointment time to ensure the full allotted time for your session. Allow time for disrobing & dressing, as well as communicating with me before & after your session. If you are late, I will do my best to have you receive your reserved time on the table. However your session may be shortened or adjusted to allow me to stay on schedule for any following clients. Fee for service(s) scheduled will apply. If I (the Massage Practitioner) am late, every attempt will be made to insure fairness by allotting more time or decreasing the fee. Cancellations & Rescheduling Please notify me as soon as possible of conflicts so other clients may utilize your appointment time. If possible, kindly give 24 hours notice to cancel or reschedule. Please give 48 hours cancellation notice for appointments 2 hours or more in length. If I need to alter appointment times, I will make every effort to inform clients in a timely manner, and will provide a 50% discount for less than 12 hours notice on client’s next appointment. I understand unexpected circumstances take place in your life. In the event you have to reschedule or cancel your appointment, I ask that you notify me a minimum of 12 hours in advance in order to avoid the Missed Appointment Fee. A single session may not be split between two people, or divided into separate appointments. Your business is highly valued and your cooperation is greatly appreciated. Missed Appointments Thank you for understanding that I make a living by reserving time especially for you. Late Cancellations & No-Shows affect my business significantly. Unless there is a valid medical emergency, Late Cancellations of less than 12 hours notice will be subject to a fee of 50% of the Standard Rate for the service (s) scheduled. No-Shows will be subject to a fee of 100% of the Standard Rate for the service (s) scheduled. Fee must be paid on or before your next appointment. No-shows and Late Cancellations for a session that is part of a Massage Package will deduct that session from the package. Please know that your cooperation & understanding is greatly appreciated. Gift Certificates, Coupons, & Sessions Certificates are good for dollar amount only; Cannot be redeemed for cash; Are transferable from one person to another, but not refundable. Clients may change the Package or Service given into a Credit and choose Services or Products to their preferences. You must have your certificate/coupon at the time of your appointment or a full charge will apply. A single session may not be split between two people, or divided into separate appointments. Prices & services are subject to change without notice. Gift certificates are honored for two years. After that date, if the gift certificate is unused, I will value 100% of the dollar amount paid for the gift certificate towards that day's current prices. Only 1 un-revokable appointment can be made for an expired gift certificate (donations). No-shows on gift certificates for single services and combination services will render certificate null and void, late cancellations will deduct 50% of certificate value. No-shows for a session on gift certificates for a Massage Package will render that session null and void. Massage Packages may be shared with another person. You may pay the difference to lengthen a session for a gift certificate or scheduled appointment. Lengthening a session should be arranged in advance. Modesty Individual privacy is highly respected. Clients are required to be properly draped throughout all services. No exceptions. As each respective section of the body is massaged, it is uncovered and then recovered as the session progresses. The genitals and /or breasts will remain covered at all times during the session and, while most people prefer to remove all clothing, you can elect to wear undergarments as you deem fit. Scope Of Practice I perform services only for which I am able (physically & emotionally), & qualified to do. I refer to appropriate specialists when work is out of my scope of practice and/or not in the client’s best interest. The treatments offered specifically exclude diagnosis, prescription, manipulation or adjustments of the human skeletal structure, or any other therapy, service or procedure which requires a license to practice physical therapy, chiropractic, osteopathy, psychotherapy, or any other profession or branch of medicine. Health Considerations It is important that you inform me, your practitioner, if you are pregnant, have serious heart ailments, diabetes, high blood pressure, skin disorders, or any other medical conditions. If you have any of the conditions listed above, I recommend you consult your physician before scheduling any massage treatments. Please insure there is enough time to discuss all health considerations prior to the beginning of the appointment. This allows me to provide you with a full session, tailored to your specific needs. I highly discourage alcohol consumption prior to or immediately following massage and body treatments. Please avoid immediate sun exposure prior to body treatments, and hot stone massage. Right of Refusal I reserve the right to refuse service to anyone. This includes but is not limited to: 1. Any individual who requests or suggests sexual or illicit services. I reserve the right to terminate a session for sexual requests, suggestions, or advances. I reserve the right to charge for the session time whether or not scheduled services were rendered. A payment IN FULL will be due, and the local law authorities may be notified. 2. Any individual who arrives for therapy under the influence of alcohol or recreational drugs. I reserve the right to charge to for the session time whether or not scheduled services were rendered. A payment in full will be due. 3. Any individual currently taking prescribed medications, or who possess a health condition, which is contraindicated for a massage. Non Discrimination All clients are treated with respect and dignity. I do not discriminate against client’s race, religion, sexual orientation, political views, age, or gender. Teens & Children: Ages 17 & younger Are required to have a parent or legal guardian sign their Client Information Forms or Submission Confirmations. Teens & children ages 15 & under are required to have a parent, or responsible adult, accompany them to their appointment. Having a parent, or responsible adult, present in the treatment room during a session is left to the parent's discretion and the client’s preference. Whatever is comfortable & acceptable for everyone is best. A parent is always welcome in the room during a child's session, and if the parent is ever to be absent for any length of time, it is made clear that there are no secrets in the therapy room -- that the child should feel free to tell the parent any and everything that happens -- and that nothing will be done in the parent's absence that would not be done in the parent's presence. Clearly, a parent should always be present during a session with a child of say, 12 or younger. Equally clearly, there is no particular need for the presence of a parent during the session of a 17-year-old, unless the client prefers it. It's the in-between ages that get tricky, and my basic policy is to have the parent and the child make the decision. If both parent and child are comfortable with the arrangements, then I am comfortable. If either of them is uncomfortable, then I am uncomfortable. Note: When a parent of a school-age child is present during a session, I suggest that she (it's usually a mom) bring a book or needlework or something else to do. Locked Door & Telephone Because I do not have a receptionist, my policy is to lock the door and to not answer my phone while we are in session. I request that you please turn off your cell phone, or pager, as well (unless there are special circumstances in which you must be contacted during this time). This allows for a peaceful session without interruption. Payments Due at the time of appointment, (unless prepaid). Cash, Checks, Debit and Credit with Visa or Master Card. Pricing does not include gratuity. Privacy Policy Away TBC collects information when needed to personally identify you. Such information is collected when, for example, you schedule an appointment, make contact regarding services, gift certificates, (etc.) or when registering as a new client. This personal information can include your name, email address, mailing address, phone number (s) and medical/health information, as applicable. Away TBC will never disclose your personal information to other companies. Client Agreement It is my choice to receive massage therapy (or body treatments). I understand that the services provided are for the well being of my body and mind, and to enhance my body’s natural healing processes. This includes stress reduction & relaxation, relief from muscular tension, spasm, or pain, and increased circulation. If I experience any pain or discomfort during a session, I will immediately inform the practitioner so that the pressure and/or techniques may be adjusted to my level of comfort. I agree to communicate with my practitioner any time I feel my well-being is being compromised. I understand that any information provided by the massage practitioner is for educational purposes only. I understand that massage practitioners are not qualified to diagnose, prescribe, or treat any physical or mental illness; nor do they prescribe pharmaceuticals or perform spinal or skeletal adjustments, and that nothing said in the course of the session should be construed as such. I realize that there is no stated or implied guarantee of effectiveness for massage and body treatments. I understand that massage or body treatments should not be construed as a substitute for medical examination, diagnosis, or treatment and that I should see a physician, or other qualified medical specialist for any mental or physical ailment of which I am aware. Because massage and body treatments should not be performed under certain medical conditions, I affirm that I have stated all my known medical conditions, and answered all questions honestly. I agree to keep the practitioner updated as to any changes in my health status, and understand there shall be no liability on the practitioner’s part should I fail to do so. I understand that the client/practitioner relationship will be held in strict confidence. I also understand that any illicit or sexually suggestive remarks or advances made by me, the client, will result in immediate termination of the session, or refusal of service by the practitioner, and I will be liable for full payment of the scheduled appointment. I have read and understand that I will be responsible for a payment of 50% of the Standard Rate for the service (s) scheduled for less than 12 hours notice of cancellation, and a payment of 100% of the Standard Rate for the service (s) schedule for neglecting to show up for an appointment. I understand and agree to comply with all of the above policies. |
